Clinician Appointments

For Appointment
Please telephone the surgery for advice of appointment. You may be fitted in for consultation without an appointment, but there can be a long wait.

Cancellation of Appointments
If you no longer require a booked appointment, please telephone to cancel it at the earliest opportunity as it may be offered to somebody else in need.

Home Visits
The most comprehensive care is available at the surgery but in the event of being too unwell to attend, the doctor may visit you at home. Please telephone to request a home visit a the earliest opportunity, preferably before 9.00am. The doctor will discuss the problem with you before deciding whether a home visit is necessary.

New Patient

Chamberlayne Road Surgery accepts new patients living within our practice catchment area. This is a circular area approx 1.5m around the practice. Please check at reception if you are in our area. You may register by presenting your NHS card, if you do not have one you will need to fill in a form called GMS1 which can be obtained from the receptionist or online.

If you have come from abroad, you must bring your Photo ID. In general if you are “ordinarily resident” in the UK you can claim free NHS treatment. All New Patients registering will be asked to provide a proof of their current address. (I.e. utility bills, Rental agreements or statements)

When you register you will be asked to fill in patient profiling questionnaire. You will be offered an appointment with one of our nurse or health care assistant for a Health Check.

When you attend for this health check, please bring list of medications you are taking. For children under 6 years age please bring Immunisation records of your children(RED BOOK).

Urgent/Out of Hours/ Emergency Appointments


If you feel your need is especially urgent and all the on the day appointments are booked please alert the receptionist who will advise you and liaise with the doctor/nurse and offer you an appointment or advice slot asap

If you have a medical problem when the Surgery is closed and you cannot wait until the next working day to be treated you can attend Urgent Care Centre at Central middlesex hospital.Acton lane.Park Royal London NW10 7NS,or urgent care centre at St Charles hospital, Exmoor street. London W10 6DZ.

For Medical Advice you can telephone 111

Alternatively, there is an A & E Department for problems like chest pain and bleeding, serious accidents. CALL 999 is needed.

For genuine medical emergencies contact the Out of Hours service Care UK  on 111

For Dental Problems out of hours call 020 3402 1312.

If you require urgent appointment and we can not offer you an appointment at the surgery on the same day we can book an appointment for you at Hub site in willesden for the same day appointment.
